Woking and Sam Beare Hospices is a charitable organisation based in Surrey committed to providing specialist palliative care to people with life limiting diseases living in North West Surrey. We work alongside other community care providers to ensure seamless care to patients with end of life care needs through education and collaborative working.

Most people living with a life limiting disease would like to stay living in their own home. So this outstanding community palliative care team work to achieve this by forming a vital link between home and the expertise of hospice care.

As part of the Community Clinical Nurse Specialist team you will liaise and work with GPs, District Nurses and all members of the MDT, providing specialist knowledge and expertise in palliative care to patients and their families/carers in their own home, care home or community hospital.

The service is provided over seven days a week 9am to 5pm with the availability of advice out-of-hours. The team accept referrals at any stage of the disease process supporting both the patient/their carers and are a resource to community health care professionals (HCP) assisting with clinical and supportive management, advance care planning and decision making towards the end of life.

You will work as part of an integrated team of nurses, within the community setting to provide a responsive service, providing specialist palliative care advice including the ability to deliver safe evidenced based symptom control advice, alongside the provision of social, emotional and spiritual care for the patient and family. You will work closely with, and act as, a resource to primary care teams and other care providers with a common aim of achieving the best quality of life possible for our patients.
