Our award winning CoSI [Co-ordinated, Safe & Integrated Hospice Care at Home] team, is a care provision by Woking & Sam Beare Hospices supported by Marie Curie and funded by Central Surrey Health.
As the lead provider of CoSI, funded by North West Surrey Clinical Commissioning Group, we are able to coordinate and deliver skilled personal and nursing care in patient's homes as their illness progresses and they require additional support to remain in their preferred place of care [typically in the patient's last 6-8 weeks of life].
The CoSI team delivers high quality end of life care and support to patients with a life limiting illness, their families, carers and those close to them at home, working with the community nursing teams and GPs across North West Surrey. Care is provided during the day and at night seven days a week, with a single point of access, which dovetails with out of hours community nursing to provide assistance, reassurance and support. This coordinated service has led to a significant reduction in unplanned, emergency admissions to acute hospitals, helping to avoid unnecessary patient distress.
Our experienced CoSI Registered Nurses and Health Care Assistants are accountable for delivering an excellent and holistic approach to patient centered care in the community.
In 2016 the CoSI team received the 'Kent Surrey Sussex Academic Health Science Network [KSS AHSN] Award for End of Life Care' and the ‘End of Life Care Champions of the Year 2016’ from The National Council for Palliative Care [NCPC].
