Community Palliative Care Team
The community palliative care teams are based at both Hospices and are involved in meeting the palliative care needs of patients and their families in their homes.
The team is made up of the following healthcare professionals:
- Clinical Nurse Specialists
- Consultant in Palliative Care / other palliative care physicians
- Physiotherapists
- Occupational Therapists
- Counsellors
- Complementary Therapists
All of the team specialise in palliative care; providing advice and support to people living with progressive life threatening illnesses and their families and carers. The team works closely alongside General Practitioners (GP's), District Nurses and other healthcare professionals who are already involved in your care.
The community palliative care team give physical, social, emotional and spiritual support with a focus on maintaining quality of life for you and your family. During home visits they will take time to listen, answer any questions you may have about your condition or the treatments you are receiving.
The community palliative care team work closely with the other members of the hospice team including the In-patient Unit and Day Care Unit.
