Hospice at Home

A Hospice at Home service will enable us to be there for support, reassurance and guidance whatever the time of day or night and offer a highly personalised combination of treatment and palliative care that normally each patient would receive in the Hospices – but in their own homes.

In November 2011 we launched an appeal to start a 24hour Hospice at Home service.

In April 2012, we launched our 24hr telephone Advice Line enabling patients or relatives to speak to a specially trained advisor who will be able to contact the appropriate experts to provide assistance. This has helped to avoid many of those distressing, unnecessary trips to hospital and will also be a direct support and comfort to those who are caring for their loved ones in their own homes.

We have extended the hours of the clinical nurse specialists to offer an 8am to 8pm service. Their main duties are to monitor the 24hr telephone advice line, visit patients when needed, make advance assessments and carry out crisis management to avoid hospital admissions.

In January 2014 our Rapid Response Service began with the employment of a Specialist Rapid Response Nurse to triage referrals to endeavour to be at someone’s bedside within two hours, which due to the support of the campaign has been funded for one year. Our next phase is to employ a Healthcare Assistant from 9.30pm – 7.30am, 365 nights per year. This will be a waking night shift primarily enabling relatives to sleep in the safe knowledge that a health care professional is available all night to care for their relative, we hope to have this service in place by Spring.
