Hospice at Home

Our Hospice at Home service enables us to be there for support, reassurance and guidance whatever the time of day or night and offer a highly personalised combination of treatment and palliative care that normally each patient would receive in the Hospices – but in their own homes.

In November 2011 we launched an appeal to start a 24hour Hospice at Home service.

In April 2012, we launched our 24hr telephone Advice Line enabling patients or relatives to speak to a specially trained advisor who will be able to contact the appropriate experts to provide assistance. This has helped to avoid many of those distressing, unnecessary trips to hospital and will also be a direct support and comfort to those who are caring for their loved ones in their own homes.

We have extended the hours of the clinical nurse specialists to offer an 8am to 8pm service 7 days a week. Their main duties are to monitor the 24hr telephone advice line, visit patients when needed, make advance assessments and carry out crisis management to avoid hospital admissions.

The community multidisciplinary team offer a Rapid Response Service, visiting patients within 2 hours if complex symptom management is required. New referrals will be seen within 24 hours if assessed by our triage nurse as urgent.

As lead providers of CoSI, funded by North West Surrey Clinical Commissioning Group, we are able to access personal and nursing care to be delivered in patients home when they have a short prognosis and require additional support to remain in their preferred place of care. The includes visits during the day and an overnight sitting service so that carers can also be supported.
