Our award winning CoSI team (Co-ordinated, Safe & Integrated Hospice Care at Home), is a partnership in care provision between Woking & Sam Beare Hospices, Princess Alice Hospice, Marie Curie and Central Surrey Health.

As the lead provider of CoSI, funded by North West Surrey Clinical Commissioning Group, we are able to coordinate and deliver skilled personal and nursing care in patients home when they have a short prognosis (typically 6-8 weeks) and require additional support to remain in their preferred place of care.

The CoSI team delivers high quality end of life care and support to patients with a life limiting illness, their families, carers and those close to them at home, working with the community nursing teams across North West Surrey. Care is provided day and night throughout the year, providing a single point of nursing contact 24/7, which dovetails with out of hours community nursing to provide assistance, reassurance and emergency support if required. This coordinated service has led to a significant reduction in unplanned, emergency admissions to acute hospitals, helping to avoid unnecessary patient distress

Our CoSI nurses are accountable for delivering an excellent and holistic approach to patient centered care in the community, which helps meet the needs of individuals.

In 2016 the CoSI team received the Kent Surrey Sussex Academic Health Science Network (KSS AHSN) Award for End of Life Care and the ‘End of Life Care Champions of the Year 2016’ from The National Council for Palliative Care.
