Making a referral
We accept referrals for patients with an advanced progressive disease and a life limiting illness prognosis whose G.P’s are within the Runneymede, West Elmbridge, Spelthorne, Surrey Heath, Woking and North Guildford areas. Commonly referred illnesses include Cancer, Motor Neurone Disease, End stage Heart failure, Parkinsons disease, Kidney disease and COPD. However this list is not exhaustive.
With patient’s consent referrals for the community teams or to our ward is made with the relevant medical information and forwarded to Woking & Sam Beare Hospices. Urgent referrals will be triaged and contacted within 24 hours but normally the same day. We usually receive referrals from patients G.P’s, District nurses, Hospital doctors and nurses and other healthcare professionals. Sometimes patients and families will call and ask about referral, but the actual referral itself needs to come from a healthcare professional to ensure it is appropriate and sent with relevant medical information.

Urgent referrals should be followed up with a telephone call to ensure it has been received and the relevant team made aware of its importance.
