Admission to the hospices is by arrangement with the medical team following a referral by the General Practitioner, Community Team, District Nurse or hospital palliative care team. Patients are generally visited at home or in hospital prior to acceptance. All referrals for admission are assessed daily, Monday to Friday. Emergency referrals will be assessed and a decision on whether admission is appropriate will be decided and prioritised.
